How it works

One pathway, three roles, one record.

    GP refers

    A short structured form captures the patient's context and consent. No access to clinical notes beyond the programme.

  2. 02

    Coordinator assigns

    Remind's programme lead reviews and pairs the patient with a trainer and gym location that fits.

  3. 03

    Trainer delivers

    Baseline at week one, structured sessions through weeks two to seven, endline at week eight. Safety checks every session.

Outcome reports flow back to the referring GP only after review by the Remind coordinator. Trainers never communicate with GPs directly.
